Metallia

In the manga, Queen Metallia (the Demon Empress) came from a dark spot on the moon, and Beryl became her first follower. Metallia was sealed away by Queen Serenity after she influenced Beryl to get the Earth people to destroy the people of the moon, thus destroying the Silver Millenium. In both the manga and anime, she doesn't have any form besides a cloud of blackness with a face.

In the Anime, it is never revealed where she came from. When they were reincarnated on Earth, Metallia ordered Beryl to bring her vast amountas of energy to restore her. She also wanted the Maboroshi no Ginzuishou, or the Illusion Siver Crystal. Metallia wanted the crystal to give her more power and to prevent the princess from sealing her away with it.
